Christ Community Health Services – Jefferson

Baptist Operation Outreach is a free mobile health care clinic for individuals or families without permanent housing. Baptist Memorial Health Care and Christ Community Health Services have operated the clinic,which is a member of the Community Alliance for the Homeless, since 2005. One of the largest health care providers for the homeless in Memphis, the mobile clinic offers free acute and primary care, disease management support,medication, prevention information and transportation to and from medical appointments in Shelby County and surrounding areas. Staffed by a licensed physician and nurse practitioner, the clinic offers patients direct and immediate access to care and referral services.
